BulletRN CASE MANAGER
MINIMUM QUALIFICATIONS
1. Is a graduate of an accredited school of professional nursing, and is currently licensed as an RN through the State Board of Nursing.

2. Prefer at least one year's clinical experience, with current home health, medical, surgical, or critical care experience.

3. Complies with accepted professional standards and principles.

4. Possesses and maintains good physical stamina and mental health. Has presented a pre-employment health clearance.

5. Must be a licensed driver with an automobile that is insured in accordance with state and/or Agency requirements and is in good working order.

6. Has satisfactory references from previous (or current) employers and/or nursing school, and professional peers.

7. Has excellent observation, good clinical judgment, and good oral and written communication skills.

8. Is self-directed with the ability to work with little supervision; has good organizational skills.

9. Is flexible and cooperative in fulfilling role obligation.

SUMMARY OF JOB RESPONSIBILITIES

The RN Case Manager coordinates all aspects of the patient's home care, interacting with other disciplines as needed, and provides professional care to the patient as indicated by agency policies, State and Federal laws and regulations. The RN case manager usually performs the admission visit to the patient in the home. The RN case manager determines patient's eligibility for home care services and develops the plan of care to be followed. The RN case manager completes all paperwork necessary for agency policies and federal/state laws.

SUMMARY OF JOB RESPONSIBILITIES

The registered nurse makes the initial evaluation visit, regularly re-evaluates the patient's nursing needs, initiates the plan of care and necessary revisions, furnishes those services requiring substantial and specialized nursing skill, initiates appropriate preventive and rehabilitative nursing procedures, prepares clinical and progress notes, coordinates services, informs the physician and other personnel of changes in the patient's condition and needs, counsels the patient and family in meeting nursing and related needs, participates in in-service programs, and supervises and teaches other nursing personnel.

1. Is a graduate of an accredited practical nurse or vocational nursing program.

2. Is currently licensed as a licensed practical nurse or licensed vocational nurse in the State.

3. Prefer one year nursing experience. Community/home health or medical/surgical experience is preferred.

4. Complies with accepted professional standards and practice.

5. Possesses and maintains good physical stamina and mental health. Has presented a pre-employment health clearance.

6. Is self-directed with the ability to work with little supervision; has good organizational skills.

7. Is flexible and cooperative in fulfilling role obligation.

8. Must be a licensed driver with an automobile that is insured in accordance with state and/or Agency requirements and is in good working order.

RESPONSIBILITIES AND DUTIES

The licensed practical nurse furnishes services in accordance with agency policies, prepares clinical and progress notes, assists the physician and registered nurse in performing specialized procedures, prepares equipment and materials for treatments observing aseptic technique as required; and assists the patient in learning appropriate self-care techniques.

MINIMUM QUALIFICATIONS
1. Is certified by the State Department of Health and is listed on the Home Health Aide registry.

2. Is in good standing with the State Nurse Aide registry as a Certified Home Health Aide.

4. Is at least 18 years of age.

5. Possesses and maintains good physical stamina and mental health. Prior to patient contact, presents a pre-employment health clearance.

6. Has ability to read and follow written instructions and document care given.

7. Is self-directing with the ability to work with little direct supervision.

8. Demonstrates empathy for the needs of the ill, injured, frail and the impaired.

9. Demonstrates flexible and cooperation in fulfilling role obligation.

10. Demonstrates tact, patience, and good personal hygiene.

11. Licensed driver with automobile that is insured in accordance with Agency requirements and is in good working order.

SUMMARY OF JOB DESCRIPTION

The home health aide provides services that are ordered by the physician in the plan of care. The duties of a home health aide include the provision of hands-on personal care, performance of simple procedures as an extension of therapy or nursing services, assistance in ambulation or exercises, and assistance in administering medications that are ordinarily self-administered.

3. Prefer at least 1-year nursing experience. Community/home health or medical/surgical experience is preferred.

6. Is self-directed with the ability to work with little supervision; has good organizational skills.

7. Is flexible and cooperative in fulfilling role obligations.

9. Has excellent observation and communication skills.

10. Ability to politely answer phones in a proper and orderly fashion

SUMMARY OF JOB RESPONSIBILITIES

Responsible for organizing, updating, and filing clinical documentation. Gathers and completes all clerical and generic portions of materials necessary for admission, recertification, and discharge of patients. Prepares, reviews and disassembles patient records after the patient has been discharged. Answers phone calls when the Supervisor and/or Case Manager is out of the office, or otherwise unavailable. Makes patient home visits as needed.

MINIMUM QUALIFICATIONS
1. Is computer literate, preferably in home health or similar operation.

2. Has a minimum of one-year experience, or graduation from a one or two year business college.

3. Can type at least 50 words per minute. Has ability to use a variety of office machines and office software programs.

4. Knowledge of medical terminology preferred.

5. Is at least eighteen years of age.

SUMMARY OF JOB RESPONSIBILITIES

The secretary/receptionist is responsible for data entry, printing, coding, and report generations of all physician orders and all information related to patient care. Is responsible for routine clerical matters and assuring that documents flow in a coordinated and efficient manner.

MINIMUM QUALIFICATIONS
1. Prefer BA in Marketing or related field or 3 years sales/marketing experience in health related field.

SUMMARY OF JOB RESPONSIBILITIES

The Community Education Coordinator is responsible for the coordination and execution of activities that increase awareness of the Agency and its services. Effectiveness of the Community Education Coordinator is measured by the goals accomplished in the office represented by the Community Education Coordinator. Responsible for developing and coordinating all clinics and other marketing activities.
